We will be participating in Easter Seals charity softball game on Sunday, August 15, 1999. The whole point of the game is we will get to play an officiated game, but we need to get sponsors and collect at least $350 as a team. I would like to ask everybody participating to collect at least $50 for the game. I have an on-line version of the Sponsor Form, which you can download. The form also contains information about the softball game and Easter Seals; please take a moment of your time to read it.

Please note that these forms are for your record only; you do not need to turn in these forms. All you do need to turn in is the money you have collected; checks can be written payable to Easter Seals. The money is due the day of the game; however, more money can be collected and be sent to Easter Seals after the game as well.
